Home To Home ServiceParents/guardians/families/whanau are important people and sometimes need to have a break from the 24 hour care that a child or young person with a mild to moderate disability requires.
The 'Home to Home' service has been designed to provide suuport or 'time out' for a child or young person with a disability in their own home or the support person's home. Shared care provides an opportunity for a child or young person with a disability to make new friendships outside the family home.
Shared Support Home to Home is:A service where all involved believe that they can work closely together to provide the best possible support.
· Thorough screening of carers (and others in the carers home) prior to registration with 'Home to Home'
This is achieved through:
· a series of interviews
· a police check
· a medical check
· a check with Child Youth & Family
· two references
· Training and support for carers
· A careful matching process prior to placement, which involves families/whanau, the carer and the co-ordinator
· Regular monitoring and ongoing support for carers and families to ensure the needs of the parties are being addressed
· Standards developed to ensure that the best possible support is being provided
This service is funded for children 0-17 years with low to moderate needs. Carer/support persons are completely funded by the Ministry of Health 'Carer Support Scheme'.
Eligibility for allocation is determined through a Needs Assessment. This assessment will determine the number of Carer support Days allocated to meet the needs of the parents/guardians/family/whanau.
There is no charge involved to either party when registering with 'Home to Home' to access this service.
Carer Support Service 'Home to Home'
· Carer Support assists parents/guardians/families/whanau
· This service provides a home environment for people who need relief support ('time out')
· Ensures that carers and others in the home providing the shared care/support are assessed as suitable and compatible to the families and the person with the disability
· Close monitoring and evaluation is provided by the 'Home to Home' Co-ordinator to endure that the needs of all the parties are addressed
· Carers attend training, which includes First Aid and Disability Awareness, to ensure that the best possible services are given
